Dollar General No. 4094, 5404 Fourth St. — observed soiled racks in cooling unit. Corrected on site.

CRITICAL VIOLATIONS

Petro Stop, 6425 19th St. — Observed no paper towels at hand sink. Observed missing light covers in kitchen area. Observed no cover on restroom trash can. Correct by next routine inspection.

Dollar General No. 10560, 1815 Milwaukee Ave. — Observed sharply dented cans. Observed soiled racks in cooling unit. Corrected on site.

Los Amigos Restaurant, 5001 34th St. — Cold hold food held at improper temperatures. Observed possible cross-contamination. Observed clean plates being stored on soiled storage rack. Observed soiled deflector plate in ice machine. Inadequate date-marking systems. Observed several damaged or broken kitchen utensils, plates and containers. Observed reach-in freezer taped shut and in need of repair. Observed personal employee beverage without lid and straw. Observed inadequate storage of sanitized wiping cloths. Observed dripping grease on vent hood. Observed mop being stored in mop bucket while not in use. Observed soiled and damaged floors, walls and ceilings. Correct by next routine inspection.
